Ongoing Coordination

 

Elder_wWalkerSon.jpgOur professionals will continuously manage and monitor the situation at home to ensure that the right resources and services are being utilized, as desired by the family and loved one. We will keep in close contact with the family to keep them informed, and alert them to any changes that may occur.

 

Implementation could include:

  • Ongoing care coordination and management of services
  • Serving as a liaison to out-of-town family members
  • Arranging and monitoring in-home assistance
  • Providing background and reference checks for private caregivers
  • Crisis intervention (i.e. hospital, nursing home)
  • Reviewing financial, legal or medical issues and offering trustworthy referrals
  • Alternative living arrangement assessment and planning
  • Counseling, support and family mediation
  • Accompanying to physicians appointments and providing written provider recommendations
  • Emergency availability via telephone 24/7
  • Securing eligible healthcare benefits
  • Providing companion visits
  • Concierge services such as shopping, pet care, transportation, etc.
