Expert Assessment
First, we will send an expert Care Manager to the home of your loved one. We will meet with the primary caregiver along with the senior to develop a complete and thorough individual analysis, which identifies risks and strengths with a holistic approach that considers medical, physical, cognitive, emotional, social, psychological and spiritual needs. Assessment can be tailored to the specific situation.
